How to Order a New Mailbox:

1. Fill out the form below and click Print.

2. A Mailbox Agreement form will be printed for you to fill out at your convenience.

3. A Mail Boxes Etc. representative will contact you to answer any questions.

4. Download, and complete a copy of the Post Office 1583 form- click here
5. Bring both completed Forms to the store and Open your New Mailbox.
 
Two forms of ID will be required-Contact this store for details of acceptable ID's.
