How to Order a New Mailbox:

1. Fill out the Order a New Mailbox section below and then click Print.

2. A Mailbox Agreement Form will be printed.

3. You will also need to complete a Post Office - 1583 Form.

4. To expedite the process, also fill out a Credit Card Authorization Form.

5. To save time fax all three forms along with copies of two ID's to 305-267-3001 or scan and e-mail them to mbe1680@mbemail.com.  We will contact you if we have any questions.  You can also bring the forms and ID's to the store.
